Background: Within an Event Report, there are three fields that below the risk assessment. These fields align to the options within the Admin Menu 'Risk Assessment Codes' as follows (shown as Event Report Heading = Risk Assessment Code): 'Notify' = 'Reporting Obligation'; 'Deadline' = 'Notes'; and 'Coordinator' = 'Investigation Reqd'. Within the Admin Menu, you can update the values for these fields based on overall risk level. Request for Change: Industry, as well as legislation, has started to move away from risk level, to more focus on actual/potential consequence. Currently, the values can only be configured for each risk level, which makes it difficult to align internal procedures with these values. As per image attached, you can see a screenshot from our Incident Management Procedure, which defines the investigation requirements etc. in line with consequence level. Can it be requested that INX modify the field structure and methodology to align with consequence level, instead of overall risk level. This type of change would also expand to other parts of the system, such as event securities, which I know were requested for implementation almost 15 years ago. -------------------------- Secondly, I would also recommend that INX align the Terms in the Admin Menu, with the actual event report labels - very confusing (I know that they can be changed using globalisation, but it should be aligned...).

Create a tab that records field changes after an event has been submitted with time, date, changes made (old data-new data and by who.

We are requesting the ability to scan a unique QR code and it would open one of the checklists we have built in the app which is specific to the QR code.

Submitters do not get notified when events are closed and therefore have no oversight of the event they reported. Create an option for events to email the submitter when event is closed.

Background: The AXTAT Reporting requirement no longer meets industry requirements in WA. We are now required to report quarterly, but the option to run a quarterly report is now disabled. Request for Change: We need to be able to run the report quarterly. In addition, we also need to update the report from "AXTAT Monthly Status" to "Regulator Status Report" as it has not been referred to as AXTAT for years. Added 11/04/2023 - Also added screenshots from the DMIRS SRS System - this is the template we have to submit for each mining operation (each quarter).

Allow for risk registers to be referenced (by hyperlink /dropdown selection) e.g Unwanted event can become Cause or Consequence of another register.

Its important to see who uses the system especially with staff that moan about not finding documents or how to address actions etc. If I could see their activity I could potentially identify a root cause.

This symbol is commonly used by the users at our sites. It would be helpful to get this fixed.

Due to the number of contractors working for us, we generally do not add contractors to InControl. However, when one of them has an injury and we need to record it, we have to create a temporary profile for them, record their injury details and then make them inactive. It would be much more efficient if another field is added to the injury form called Name (if not list). This does appear on the Report Details, so if this could be duplicated to the Injury form it would be ideal. The majority of our contractors do not use InControl and are only temporary and therefore do not need an active profile.
